Section 7: Affixing of photograph to medical certificate

A photograph of the applicant shall be affixed at the appropriate place shown in 21[Form 1-A] and the registered medical practitioner shall affix his signature and seal to the said photograph in such a manner that the signature and the seal appear partly on the photograph and partly on the form of the medical certificate:
