An insured woman, who decides to give notice of pregnancy before confinement, shall give such notice in Form 17 to the appropriate Branch Office, by post or otherwise and shall submit, together with such notice, a certificate of pregnancy in Form 17 given in accordance with these regulations on a date not earlier than seven days before the date on which such notice is given.
