An application for registration as a citizen of India under clause

(d) of sub-section (1) of section 5, for a minor child of a person who is a citizen of India, shall not be entertained unless –

(a) the application is made in Form IV; and

(b) the parent of such minor child declares that he is the legal guardian of the child.
