l2) Every affidavit sha!l ,conclude as follows :-:

·· 1 do swear in U1e name of God U1at this is my name and solemnly affinn. thumb impression/Signature and U1at the C'0lltents of this affidavit are true. 1 further swear/solemnly affinn tJ1at what is stated in paragraph (give numbers) is true to my person~! knowledge and what is stated in para­ ~raphs (gi,•e numbers) is ·true to my infonnation ar1d is believed by me to be true.·
