(1) ज्ञापन सू᭯म, लघु और मध्यम उ᳒म मंᮢालय, भारत सरकार की वेबसाइट http://udyogaadhaar.gov.in पर ऑनलाइन फाइल िकया जाएगा;
(2) उᲦ अिधिनयम की धारा 7 मᱶ उिल्लिखत सीमा से नीचे के संयᮢ एवं मशीनरी मᱶ कुल िनवेश से वस्तुᲐ के िविनमार्ण (मैन्यूफैक्चᳳरंग) अथवा सेवा दनेे मᱶ लगे ᮧत्येक सू᭯म, लघु और मध्यम उ᳒म ज्ञापन फाइल करेगा;
(3) िविनमार्ण अथवा सेवा अथवा दोनᲂ सिहत एक से अिधक ᮰ेिणयᲂ को इस ज्ञापन मᱶ िन᳸दर᳥् िकया जा सकता ह;ै 2 THE GAZETTE OF INDIA : EXTRAORDINARY [PART II—SEC. 3(ii)]
(4) स्वािमत्व-फमर् के मामले मᱶ स्वामी, साझेदारी फमर् के मामले मᱶ ᮧबंध साझेदार तथा िहदं ू अिवभािजत पिरवार (एचयूएफ) के मामले मᱶ कतार् ज्ञापन मᱶ अपनी आधार संख्या दगेा;
(5) कम्पनी अथवा सीिमत दयेता साझेदारी (िलिमटेड लायिबिलटी पाटर्नरिशप) अथवा सहकारी सिमित अथवा सोसाइटी अथवा न्यास के मामले मᱶ ᮧािधकृत (ऑथोराइज्ड) हस्ताक्षरी की आधार संख्या के साथ संगठन की स्थायी खाता संख्या (पैन) का ज्ञापन मᱶ उल्लेख िकया जाएगा;
(6) नये ज्ञापन के सत्यापन तथा मौजूदा ज्ञापन के संशोधन के िलए एक बारगी पासवडर् (ओटीपी) आवेदक की आधार संख्या से जुड़े मोबाइल फोन पर भेजा जाएगा और आवेदक ज्ञापन ᮧस्तुत करने से पूवर् फॉमर् मᱶ ओटीपी दजर् करेगा;
(7) ज्ञापन स्वघोषणा आधार पर फाइल िकया जाएगा तथा ऐसी फाइिलंग के समय कोई अितिरᲦ दस्तावेज अपलोड िकया जाना अपेिक्षत नहᱭ ह:ै बशतᱸ िक केन्ᮤ सरकार अथवा राज्य सरकार अथवा इसके िलए ᮧािधकृत ऐसा ᳞िᲦ जहाँ कहᱭ आवश्यक हो, ज्ञापन मᱶ दी गई सूचना का दस्तावेजी सबूत मांग सकता ह;ै
(8) फॉमर् II मᱶ उ᳒ोग आधार पंजीकरण ᮧमाणपᮢ सृिजत कर ज्ञापन मᱶ िदए गए ई-मेल पते पर मेल िकया जाएगा;
(9) उ᳒ोग आधार ज्ञापन फाइल करने के िलए कोई शुल्क नहᱭ होगा;
(10) यिद आवेदक अथवा ᮧािधकृत हस्ताक्षरी के पास आधार संख्या नहᱭ ह ैअथवा यिद िकसी कारण से आवेदक के िलए ऑनलाइन फाइिलंग संभव नहᱭ ह ैतो वहाँ पर िविधवत् भरे हुए फॉमर् I की हाडर्कॉपी िन᳜िलिखत वैकिल्पक दस्तावेज और पहचान पᮢ के साथ सम्बिन्धत िजला उ᳒ोग केन्ᮤ (डी आई सी) अथवा िवकास आयुᲦ सू᭯म, लघु और मध्यम उ᳒म के अधीनस्थ सू᭯म, लघु और मध्यम उ᳒म-िवकास संस्थान (एमएसएमईडीआई) कायार्लय मᱶ ᮧस्तुत की जाएगी (क) (i) आधार पंजीकरण आईडी िस्लप; अथवा
(ii) आधार पंजीकरण के िलए िकए गए अनुरोध की ᮧित;
(ख) आवेदक या ᮧािधकृत हस्ताक्षरी के िन᳜िलिखत दस्तावेजᲂ मᱶ से कोई, नामत:-
(i) बᱹक पास बुक की फोटोकापी; अथवा
(ii) मतदाता पहचान पᮢ काडर्; अथवा

(iv) ᮟाइिवंग लाइसᱶस; अथवा
(v) पैन काडर्; अथवा

(11) कोई भी उ᳒म एक से अिधक ज्ञापन फाइल नहᱭ करेगा;
(12) पते मᱶ पिरवतर्न अथवा उ᳒मᲂ ᳇ारा कायर्कलापᲂ मᱶ पिरवधर्न (एडीशन) अथवा िवलोपन (िडलीशन) अथवा अन्य पिरवतर्नᲂ के मामले मᱶ स्वामी अथवा ᮧबंध साझेदार अथवा ᮧािधकृत ᳞िᲦ सू᭯म, लघु और मध्यम उ᳒म मंᮢालय की वेबसाइट http://udyogaadhaar.gov.in पर उपलब्ध ऑनलाइन उ᳒ोग आधार ज्ञापन की फाइिलंग के िलए िदशा-िनदᱷशᲂ के अनुसार िव᳒मान ज्ञापन के संशोधन के िलए आवेदन कर सकता ह;ै

(1) The memorandum shall be filed online on the website of the Ministry of Micro, Small and Medium Enterprises, Government of India at http://udyogaadhaar.gov.in;
(2) Every micro, small and medium enterprise engaged in the manufacturing of goods or rendering of services with total investment in plant and machinery within the limit specified in section 7 of the said Act, shall file the memorandum;
(3) Any number of activities including manufacturing or service or both may be specified in the memorandum;
(4) The proprietor in the case of a proprietorship firm, managing partner in the case of a partnership firm and a karta in the case of a Hindu Undivided Family(HUF), shall provide his Aadhaar Number in the memorandum;
(5) In case of a company or a limited liability partnership or a cooperative society or a society or a trust, Permanent Account Number(PAN) of the organisation along with Aadhaar number of the authorised signatory shall be specified in the memorandum;
(6) A one time password (OTP) for verification of the fresh memorandum as well as amending of the existing memorandum shall be sent on mobile phone linked to the Aadhaar number of the applicant and the applicant shall enter the OTP in the form before submitting the memorandum;
(7) The memorandum shall be filed on self certification basis and no additional document is required to be uploaded at the time of such filing:
Provided that the Central Government or the State Government or such person as may be authorised in this behalf may seek documentary proof of the information provided in the memorandum, wherever necessary;
(8) Udyog Aadhaar Registration Certificate in Form II shall be generated and mailed to the email address provided in the memorandum;
(9) There shall be no fee for filing the Udyog Aadhaar Memorandum;
(10) In case an applicant or the authorised signatory does not have Aadhaar number or in cases where online filing is not possible for any reason, a hard copy of the duly filled Form I, shall be submitted to the concerned District Industries Center (DIC) or to the Office of the Micro, Small and Medium Enterprise-Development Institute (MSME-DI) under the Development Commissioner, MSME alongwith the following documents as alternative and viable means of identification:
(a) (i) Aadhaar enrolment ID slip; or
(ii) a copy of the request made for Aadhaar enrolment; or
(b) any of the following documents of the applicant or the authorised signatory, namely:—
(i) Bank photo passbook; or
(ii) voter ID Card; or
(iii) passport; or
(iv) driving license; or
(v) PAN card; or
(vi) employee photo identity card issued by the Government.
(11) No enterprise shall file more than one memorandum;
(12) In case of change in address or addition or deletion of activities by the enterprises or for other changes, the proprietor or the managing partner or an authorised person may apply for amendment of the existing memorandum in accordance with the guidelines for filing the online Udyog Aadhaar Memorandum provided in the website of the Ministry of Micro, Small and Medium Enterprises, at http://udyogaadhaar.gov.in;
(13) The General Manager of the District Industries Center of the concerned district shall undertake enquiry and verification of the memorandum submitted by the enterprise and in case of any discrepancy, issue a notice to the enterprise, giving it an opportunity to present its case and based on the findings, he may amend the memorandum or cancel the Registration Certificate if already issued.

Note:
(i) Enterprises that have filed EM-II or SSI registration, shall not be required to file UAM but may file it, if they so desire.
(ii) EM-I has been stopped and after establishing a unit, an enterprise has to file UAM which replace SSI registration and EM-II. Regulatory authorities, banks, utility etc. should not insist on EM-I.
