(1) If a license under section 4 is lost, stolen or destroyed, the license shall forthwith report the matter to the competent authority by whom the license was issued or, as the case may be, last renewed and may make an application to that authority for the issue of a duplicate license .
(2) The application shall be accompanied by a treasury receipt showing that the fee for the issue of a duplicate as specified in rule 8 has been paid.
(3) On receipt of the application, the competent authority shall grant to the applicant a duplicate copy of the license duly stamped “duplicate” in red ink.
