Section 9: Change of addreses

The Karnataka. Nurses Midwives and Health visitors Rules, 1964State Rules of Karnataka · 1962

Every registered person shall immediately give notice to the Registrar of any change of name or change of permanent address and the Registrar shall amend the Register accordingly.

Along with the notice of change of name shall be forwarded documentary evidence in support of the change for recording the change of name in the Register.
