(a) Application for the grant of licence under these rules shall be submitted in the Forms mentioned below:
Name of Form Nature of business in milk or milk products
(i) Form 'A' Schedule 1 For setting up a dairy or processing unit under Section 11(1) of the Act.
(ii) Form 'B' Schedule 1 For carrying on business in milk or milk products as Dairy under Section 11 (2) of the Act.
(iii) Form 'C' Schedule 1 For carrying on business in milk or milk products as “Manufacturer’’ under Section 11 (2) of the Act.
(iv) Form 'D' Schedule 1 For carrying on business in milk or milk products as “Processor’’ under Section 11 (2) of the Act.
(v) Form 'E' Schedule 1 To collect, carry or transport milk or milk products within reserved area or to any place outside such area under Section 13 of the Act.
b. The application shall be presented by the applicant in person or sent through the medium of post office or otherwise, to the Licensing 13 Authority, having jurisdiction in respect of the place where the applicant has his business premises or where the area of his operation is located.
c. The application shall contain all such information as is necessary for the consideration of the application.
