(a) Licences granted under the Act and rules framed thereunder will be issued in the forms mentioned below:

(i) Form I Schedule 2 Licence for setting up a dairy or milk processing unit under Section 11(1) of the Act.
(ii) Form II Schedule 2 Licence for carrying on business in milk or milk products as "Dairy" issued under Section 11(2) of die Act.
(iii) Form III Schedule 2 Licence for carrying on business in milk or milk products as "Manufacturer" issued under Section 11(2) of the Act.
(iv) Form IV Schedule 2 Licence for carrying on business in milk or milk products as "Processor" issued under Section 11(2) of the Act.
(v) Form V Schedule 2 Licence for carry on business to collect, carry or transport milk or milk products within a reserved area or outside such area under Section 13 of the Act.
(b) The licences in Form I, Form II, Form III and Form IV will be granted for a period of one year or more as decided by the Licensing Authority from time to time. Licence under Form V will be granted for a period specified by the Licensing Authority in each case.
19. Renewal of licences 1[(1) Every licence may, at its expiration, be renewed by the Licensing Authority for a period not exceeding five years.]
2) Subject to sub-rule (1), every licence should ordinarily be renewed before the expiry of the date of the licence.
3) The Licensing Authority may consider an application for renewal of a licence if the period between the date of its expiry and the date of application does not exceed three months. The applicant shall have in such a case to pay penalty at the rate of Rs. 200 for licences Forms I and II and Rs. 50 per calendar month for licences in Forms III, IV and V.
