4[The undertaking in the form of an affidavit sworn before a Notary or a Judicial/Executive Magistrate] shall be in the following form:

I, Shri/Smt.lKumari son/wife/daughter of ............ residing at do hereby swear and undertake to abide by the code of conduct of registered valuer-surveyor as set forth in the 5[West Bengal Valuation Board] (Registration of Valuer-Surveyor) Rules, 1993, to the best of my ability and conscience.
