· Applications under sub-section (2) of section 15 by or on behalf or an employed person or group of employed persons or an Inspector or any other person acting with the" permission or the authority appointed under sub section (I) of section 15, shall be made in duplicate in Form VII, Form VIII or Form IX, as the case may be, one copy of which sha ll bear such court fee as may be prescribed.
