(1) The sample of medical device or portion thereof sent by Medical Device Officer to the Medical Device Testing Officer for test or evaluation under subsection (4) of section 23 of the Act shall be sent by registered post or by hand in a sealed packet, enclosed together with a memorandum in Form MD34, in an outer cover addressed to the Medical Device Testing Officer.

(2) A copy of the memorandum and a specimen impression of the seal used to seal the packet shall be sent to the Medical Device Testing Officer separately by registered post or handed over by hand and a copy of the memorandum shall be endorsed to the manufacturer.
