Save as otherwise provided in these i1.Iles . every complaint under the Act shall be made. as .far as poss ible. in Fom1 A and shall contain the following particulars name­ ly:- (al The name and address of the complainant.

(bl TI1e name. official designation a11d address of the public func­ tionary ~a.inst whom a complaint stating the allegation is made.

{cl The detail s of the content of. the action complained and of the allegation .

(d) A Statement that the eomplainant has not for the same matter resorted to any o·tJ1er remedy by way of proceedings before _any tribunal or court. ef law or any other authority empowered to decide that matter.

If U1e complainant has resorted to any such remedy. the designation of U1e tribunal or court or authority. as the case may be. before which s u~h proceedings were instituted. the _date on which they were in.­ stit11ted . the number given to sucn proceed.ings. if the proceedings are disposed of the result of such proceedings and if the proceedings are pending. the s tage at which pending should be stated.
