(2) The appeal shall be in Form-5 and shall be signed by the appellai1t and presented by him in person or through his authori,sed representative. The memorandum of appeal shall be accompanied by the original or certified copy of the order or decision against which the appeal is· preferred.

{3) The appeal shall be submitted in triplicate.

( 4) No appeal shall be entertained unless a copy thereof has been served by the appellant to .the Town Vending Committee which had made the order appealed against, prior to filing of such appeal and proof of such · service shall be filed along with the appeal.
