(1) The statement required to be sent or delivered to the Registrar under section 58 shall be filed online in Form-A appended to these rules and shall be witnessed in the manner specified therein.
(2) On receipt of Form-A along with the fee, e-Signed certificate in the format given below shall be generated and same may be downloaded by applicant using his/her login credentials.
